TUMOR ODONTOGÊNICO ADENOMATÓIDE EM REGIÃO POSTERIOR DE MANDÍBULA: RELATO DE CASO ADENOMATOID ODONTOGENIC TUMOR IN THE POSTERIOR REGION OF THE MANDIBLE: A CASE REPORT

Resumo

Introdução: O tumor odontogênico adenomatóide (TOA) é uma neoplasia epitelial benigna, assintomática, de comportamento não agressivo e crescimento lento. Está associado a um dente incluso e afeta principalmente a região anterior da maxila. O seu diagnóstico diferencial deve ser bem conduzido para evitar cirurgias ablativas extensas. Objetivo: Relatar um caso clínico de tumor odontogênico adenomatóide atípico, associado a um dente impactado na região posterior de mandíbula. Descrição de caso: Paciente do sexo feminino, 74 anos, procurou o serviço de cirurgia bucomaxilofacial do Hospital Universitário Lauro Wanderley (HULW) queixando-se de dor na região posterior de mandíbula, lado esquerdo. Na avaliação física, a mucosa oral apresentava-se com discreta coloração arroxeada e um pequeno aumento de volume na região de rebordo alveolar. Ao exame radiográfico, constatou-se uma lesão radiolúcida unilocular, bem delimitada, no ramo mandibular e associada a um terceiro molar incluso. Foi realizada biópsia incisional da região, sob anestesia local e o material coletado foi encaminhado para análise histopatológica que confirmou a hipótese diagnóstica de TOA folicular intraósseo. Como tratamento, inicialmente, instalou-se um dispositivo descompressor na região que permaneceu por nove meses. Após a regressão da lesão, foi realizada exodontia do elemento dentário associada à enucleação cirúrgica do cisto e coronectomia do segundo molar devido ao risco de fratura mandibular. Conclusão: Como forma terapêutica eminente do TOA, a enucleação cirúrgica conservadora e a curetagem da lesão é o tratamento de escolha, com excelente prognóstico.

Introduction: The Adenomatoid Odontogenic Tumor (AOT) is a benign non-aggressive, asymptomatic neoplasm with slow growth. It is often associated with embedded teeth and mainly affects the anterior region of the maxilla. Purpose: To report a case of atypical adenomatoid odontogenic, associated with an impacted tooth in the posterior region of the mandible. Case description: The patient was a female, 74 years old, looked for dental services in Hospital Universitário Lauro Wanderley (HULW), complaining of pain in the posterior region of the left mandible. The oral mucosa presented itself as a slight purple color and a small increase in volume in the alveolar ridge region in the physical evaluation. In the radiographic exam, a radiolucent lesion was found on the left side of the mandibular branch, unilocular, well delimited, associated with an included the third molar, exceeding the cementoenamel line. It was performed an incisional biopsy of the region, under local anesthesia, and the collected material and sent for histopathological analysis confirmed an adenomatoid odontogenic tumor's diagnostic hypothesis. As a treatment, initially, a decompression device was installed in the region that remains for nine months. After the lesion regressed, the tooth extraction was associated with surgical cyst enucleation and a second molar coronectomy due to mandibular fracture risk. Conclusion: As an eminent therapeutic form of adenomatoid odontogenic tumor, conservative surgical enucleation, and curettage of the lesion treat choice with an excellent prognosis.
