Resumo

Com o advento da Implantodontia moderna, tem sido cada vez maior
a utilização de enxertos ósseos frente à perda fisiológica, traumática
ou patológica a que os ossos maxilares são submetidos. Objetivo: O
presente trabalho visa, por meio de um relato de caso clínico, abordar
a possibilidade de associação de enxertos ósseos na reconstrução de
defeito ósseo maxilar para instalação de implantes dentários. Descrição
do Caso: Paciente M.A.G, gênero feminino, faioderma, compareceu
ao consultório particular encaminhada pela clínica de endodontia,
para exodontia da unidade 22 devido à fratura radicular. Ao
exame clínico, observou-se a presença de fístula ativa associada a esta
unidade, e através de exame de imagem verificou-se área hipodensa
compatível com extensa lesão periapical. O plano de tratamento incluiu
exodontia da unidade fraturada, enucleação da lesão associada,
reconstrução do defeito ósseo com enxerto autógeno e liofilizado bovino
e posterior reabilitação com implante dentário e prótese sobre
implante. O caso encontra-se com 3 anos de acompanhamento, com
resultado estético e funcional satisfatório. Conclusão: Diferentes tipos
de biomateriais agregados apresentam-se como uma técnica alternativa
eficaz e de menor morbidade, possibilitando o reparo de defeitos
críticos em osso maxilar para posterior reabilitação.

ABSTRACT

With the advent of modern Implantology, there has been increasing
the use of bone grafts by the physiological, traumatic or pathological
loss to which the maxillary bones are submitted. Objective: This study
aims, through a case report, to describe the possibility of association
of bone grafts in the reconstruction of maxillary bone defect for dental
implants. Case Description: The patient M.A.G, female, faioderm,
attended the private practice sent by the endodontic clinic for unit
22 extraction due to dental root fracture. At clinical examination was
observed the presence of active fistula associated with the unit, and
through image examination was verified hypodense area, compatible
with extensive periapical lesion. The treatment plan included fracture
unit extraction, enucleation of the associated lesion, reconstruction of
the bone defect with autogenous graft and lyophilized bovine bone,
and subsequent rehabilitation with dental implants and prostheses.

The case has 3 years of follow-up with satisfactory aesthetic and functional
results. Conclusion: Different types of aggregated biomaterials
are presented as an effective alternative technique, of lower morbidity,
allowing the repair of critical defects in maxillary bone for later rehabilitation.
